Thanks for your replies. I will try to call friends in Naples and see if they will drop it off at a TIM store after I mail it to them. It is annoying, since they took all this when I get the SIM.

I have found an address to send the TIM form to (I think), but I have a little problem with the form. This part, I do not understand what they want to be filled in on the line:
codice fiscale XXX XXX XXXXXXXXXX
recapito telefonico________________________________________ __________,
dichiara di essere possessore dell'utenze radiomobile numero XXX XXXXXXX.
What would go on the line after the codice fiscale?

Freetranslation.com comes up with this:
I deliver telephone________________,declares to be possessor of the subscription radiomobile number.
Does my name go in there as the user of the number?

"recapito telefonico" means a phone number of yours on which you can be called to if they need anything from you.
