Voicestick on a Nokia SIP phone

I have tried forever to setup a Voicestick SIP account on my E51, however some of the details are wrong, and their CC won't give me all the details I need.

The settings I need are as follows:

Public user name: is it +1xxxxxxxx@
Use security: Yes or no?

In proxy server menu:
Proxy server address:
Realm:
User name: +1xxxxxxxx or 1xxxxxxxx@?
Allow loose routing: Yes or no?
Transport type: Auto, UDP or TCP?
Port: 80 or 5060?

For Registrar server menu:
Registrar server address:
Realm:
User name: +1xxxxxxxx or 1xxxxxxxx@?
Transport type: Auto, UDP or TCP?
Port: 80 or 5060?
