I've had a number of prepaid and postpaid carriers.
I've also lived through a number of rebrandings.
These are the rebrandings I've survived (in my areas: Boston USA and Greece):
USA:
Omnipoint --> Voicestream --> T-Mobile (used 2000 to 2005)
Cellular One --> AT&T wireless --> Cingular --> at&t mobility (current)
Greece:
Telestet --> TIM --> Wind (not currently with Telestet, used 2000 to 2003)
PanaFon --> PanaFon-Vodafone --> Vodafone (Used 2002 to 2005)
How about you? ;-)
In Italy we just had the rebrand
Omnitel -> Omnitel-Vodafone -> Vodafone-Omnitel -> Vodafone (every time changing all commercials and signs on shops).
Some countries have changed all operators' brands, such as:
Czech Republic:
Paegas -> T-Mobile
Oskar -> Vodafone
Eurotel Praha -> O2
Slovakia:
Eurotel Bratislava -> T-Mobile
??? (don't remeber!) -> Orange
btcellnet- 02
UK...yeah ok there was also 0ne to one-tmobile uk
no i didnt use them but there name changed...
also...easymobile uk- shimmerbright....

Japan:
J-Phone -> Vodafone -> Softbank
For the US rebranding you really need a flow chart. It's much more complicated than AdmiralAK has shown.

Slovakia:
Eurotel Bratislava -> T-Mobile
??? (don't remeber!) -> Orange
Orange SK was Globtel.
Poland
Operator rebrands:
Idea->Orange
Era->T-Mobile - should just be in progress or even completed but still pending due to "legal sabotage" from Vivendi :-P
Prepaid rebrands:
Simplus->Simplus Team->Simplus again:-D
just remembered...02 used to be btcellnet which had an online sub brand known as genie.....tch its in my sig ffs!
so it was btcellent (high street) btgenie (online) then to 02 (and again there is the online and the high street arms!

That was just my personal experience :-)
I hope someone has a flow chart somewhere (or is willing to make one) -
I find the whole consolidation thing fascinating
Beat this (Germay):
D2 Privat -> D2 Mannesmann -> D2 Privat -> D2 Vodafone -> Vodafone D2 -> Vodafone
